Our Know and Thrive Approach to Homelessness Solutions

Rather than providing independent services that only address immediate needs, a Know and Thrive approach looks at individuals through a holistic lens. MGT is operationalizing this new, evidence-based approach. It integrates and systematizes community efforts and investments to ensure those experiencing homelessness have the support and connections they need to thrive. The pillars of this approach are:

The Pathway to Human Thriving is a unifying framework and an assessment tool that helps guide clients toward stability across essential dimensions that help them achieve long-term thriving and stability.

The tool supports individuals experiencing homelessness by assessing their current needs, tracking their progress, and developing achievable goals.

The Pathway to Human Thriving contains two key components:

  • An array to understand important human needs

  • A scoring system for each of these

Individualized care plans allow case managers and clients to work together to set actionable goals related to each client’s areas of need by recording these goals and making specific plans for next steps along the path of progression.

Real-time, population-specific data that is shared across case managers, providers, and other decision makers enables communities to track progress, identify gaps, and make informed decisions about how to support individuals and families on their path toward long-term stability. MGT supports this work by assisting communities in the implementation of underlying Homeless Management Information Systems (HMIS) or other related information systems.

Building sufficient caseworker capacity; integrating care-plans and progress tracking into HMIS; securing sufficient long-term funding; and monitoring and evaluating progress are key enablers for sustainable implementation.​

Ecosystem support is essential for effective program execution –Homelessness efforts do not exist in a vacuum. MGT offers a variety of customized support services based on community-specific needs.

Our Principles

We recognize the variability in sub-populations’ challenges and needs and will apply the Pathway to Human Thriving framework to address these with a Know and Thrive system, ensuring individualized and coordinated care.​

We are conscious of the root causes and complexities of homelessness, where economic and health shocks often lead to housing loss. In most cases, these exacerbate individual challenges, turning temporary setbacks into long-term patterns.​

We expect service providers to prioritize evidence-based practices that most effectively drive progress. Efficacy rates will be tracked to facilitate shared learning and success measures will focus on successful progress, not just program completion.

We provide strong support with clear expectations. Individuals must engage in healing and follow rules to prevent harmful behaviors. Additional services will link progress on the Pathway to Human Thriving with greater access to benefits.​

We focus on long-term results, prioritizing incremental progress along the Pathway to Human Thriving over merely addressing immediate needs. Program benchmarks will evaluate how each service provider contributes to long-term success.​

Customer Highlights/Case Studies

The Know and Thrive approach, locally adapted to ‘Know-by-Name’, is transforming how Utah supports individuals and families experiencing homelessness. Backed by state and private partners, this data-driven model delivers personalized, coordinated care designed to foster long-term stability.
  • Phase 1

    Launched in July 2024, marked a shift from traditional by-name lists to a holistic Know-by-Name system—with the goal of offering individualized support to 100% of people experiencing homelessness. Led by the Utah Impact Partnership (UIP), the Utah Office of Homeless Services (OHS), and MGT, the pilot showed that person-centered care, centralized case management, and shared data can lead to more effective outcomes.
  • Phase 2

    Launched in January 2025, builds on this foundation. It emphasizes regular data reporting, prioritization alignment, and stronger cross-agency coordination. Monthly impact reports show early progress, and collaboration with service providers continues to expand.
